Summer Camp Forms

Parks & Recreation offers more than a dozen exciting summer camps programs.  Some outdoor recreation camp programs require you and your family physician to complete and emergency medical form to ensure that your child is fully protected.  Below are copies of the forms in PDF format

In addition, when possible, Parks & Recreation will try to accommodate families who wish to place their camper in the same session, time and location as another camper.  Group placement requests can only be made during the registration process either online or in person.
