Information for Business Owner’s License Transportation for Hire

• Taxi Business License Fee is $150
• Each License expires annually on May 1
• Each Vehicle Certificate of Operation is $50 (see below)
• Fees are Non-Refundable and due upon presentation of application

A penalty of twenty dollars ($20.00) shall be charged to the maker of a check returned for insufficient funds.

Requirements

• Fingerprinting Receipt (follow fingerprinting instructions)
• Rate Schedule 6” x 8” Card
• Business Registration (D/B/A), Corp., or Partnership Name, Tax ID 
• Vehicle Information (Registrations, Insurance Certificates, Inspections) 
• Names and Addresses of all Vehicle Operators 
• Certificate of Liability Insurance for Business Office Location 
• Training Course Certificate as may be Required

Fingerprinting: As of January 1, 2010, fingerprinting for new applicants will be done through a NYSDivision of Criminal Justice Services approved Statewide Vendor-Managed Civil Fingerprint Capture System. (See: Fingerprinting Instructions).

Information for Operator’s Permit Transportation for Hire  

• The Permit Fee for a Taxi Operator is $50 (in addition to Fingerprinting Fees)
• Each Permit expires annually on May 1
• The Fee is Non-Refundable and is due upon presentation of application

Requirements

• Fingerprinting Receipt (follow fingerprinting instructions)
• Doctor's Statement: A witnessed statement from a doctor licensed to practice medicine in NY State stating that applicant has been thoroughly examined within thirty (30) days of application and demonstrates no affliction of any physical or mental disease or infirmity which might make him/her an unsafe or unsatisfactory permit holder, and has no drug or alcohol dependency or use other than by prescription. Results of Separate Drug Testing Must Accompany the Doctor’s Statement.
• Photographs: Two identical (2” x 2”, head and shoulders) photographs taken no longer than sixty (60) days prior to submission of application.
• NYS Driver's License:  A copy of the applicant’s valid NYS Driver’s License clearly indicating the identification number and expiration date, with authorization for Town to conduct an examination of applicant’s driving record. License must be Class A, B, C or E.
• Training Course Certificate as may be Required.
